Our Partners

Project C3: Connecting Youth to Communities and Careers is a partnership among several Minnesota state agencies and PACER Center, a nonprofit agency serving Minnesota families of children and youth with disabilities. Partners include Pathways to Employment, the Minnesota Department of Education and the Minnesota Department of Employment and Economic Development. These organizations have partnered with various entities, such as Workforce Centers, and several high schools in Minnesota to provide internships to help local youth learn, earn, and participate in community activities.

What is Project C3: Connecting Youth to Community & Careers?

Purpose?

  • Many families are unaware of various program and services available to teens and young adults in their community and this can be challenging when transitioning from high school to the adult world of postsecondary education, employment, and independent living.
  • Through a collaborative effort, Project C3 connects communities to services in Minnesota through facilitated collaboration, resource mapping, training, and advocacy for solutions to transition, employment, and accessing post-secondary education.

Goal of Project C3

The goal of Project C3 is to improve transition outcomes for youth after high school in preparation for employment or post-secondary through training, advocacy, facilitated system collaborations, and resource mapping.

  • To help achieve this goal, Project C3 created an online tool to assist youth and young adults (with and without disabilities) in planning for their future and life after high school.
  • In an effort to make services more accessible, Project C3’s website has housed multiple resources on its site including links to: education, employment, housing, health, recreation, family supports, and an interactive resource mapping tool for youth, young adults, families, and professionals.

What is Resource Mapping? (click to open/close)

  • Resource mapping is a process of collecting and saving information from community services to an online database where the public can search for resources in employment, transportation, housing, education, recreation, and health assistance.
  • This information is available at www.c3online.org. Click on “C3 MN” to access the resource map featuring these services in Minnesota.
